The traditional form of underwriting, which incorporates both a medical and non-medical assessment of the applicant (proposed insured individual).
The applicant must fill out a comprehensive application and undergo a thorough blood and urine analysis. The insurance company may also want to examine the applicant’s motor vehicle report (MRV), request an attending physician’s statement (APS) from the applicant’s primary care physician and any specialists the applicant/insured has recently seen, and check the RxCheck prescription database for any medications the applicant is taking or has recently taken.
In addition to medical examination, the insurer might also require various tax and other financial documents to assess eligibility for a policy.
